CBT and The Matrix Model
Transitioning Lives focuses on adults navigating substance use alongside mental health concerns, providing outpatient and intensive outpatient programming for alcohol, opioid and other substance use disorders.
The program also welcomes clients continuing opioid or methadone medication, supporting recovery without requiring a pause in medication.
Treatment draws on cognitive behavioral therapy, motivational interviewing, rational emotive behavioral therapy, the Matrix Model and relapse prevention planning.
Sessions are offered in individual, group, family and couples formats, helping clients address underlying triggers while building practical tools for long term wellness.
Support for Court Involved and Trauma Affected Clients
The program serves several higher-need populations, including court referred and DUI/DWI clients, people living with HIV or AIDS and those affected by trauma or domestic violence.
