About OnTrack HOME Program
OnTrack HOME Program is a residential substance use treatment program in Medford, Oregon for expecting, pregnant and parenting mothers with children newborn to age five.
The program offers structured counseling, family therapy, case management and peer services for women navigating addiction, co-occurring mental health concerns and trauma.
The HOME Program is built on the belief that mothers shouldn't have to choose between recovery and their children, creating a family-centered path to lasting healing.
Family-Centered Recovery for Mothers
The HOME Program supports up to 24 mothers in a residential setting where they can live alongside their young children throughout treatment.
With a licensed on-site childcare center, 24/7 staffing and residential services, the program is designed around the realities of motherhood and recovery happening together.
Residents engage in a minimum of 20 hours of group counseling weekly, plus individual sessions, couples and family counseling and milieu skills training.
Proven approaches such as cognitive behavioral therapy, motivational interviewing, the Matrix Model and relapse prevention are paired with trauma-informed care.
Continuum of Care and Transition Planning
As part of OnTrack Rogue Valley's broader recovery ecosystem, the HOME Program connects mothers to outpatient services, transitional housing, peer support and discharge planning.
This continuum helps families step down into stable, independent living with ongoing support.
