Transitional Living and Aftercare
Admission begins with screening and an intensive assessment, after which residents commit to a six to 12 month, phase based recovery program.
Each of the three phases has a two month minimum and is followed by three months of aftercare with twice monthly case manager meetings.
Residents participate in community meetings and Recovery Dynamics modules, 12 step meetings and counseling while building a personal relapse prevention plan.
Reported therapeutic approaches include cognitive behavioral therapy and motivational interviewing, the Matrix Model and trauma related counseling for those with co-occurring conditions.
