Horizon House at Susquehanna Park

Men with documented chronic homelessness of over three years are admitted to Susquehanna Park by referral from an outreach team or a shelter. The center is an addiction treatment center in Philadelphia.

It has been a modified therapeutic community (a residential program in which daily life with the group is part of care). In Pennsylvania, it’s part of a system for people who are homeless and in treatment for substance use.

Susquehanna Park has 25 beds, and a stay runs one year to eighteen months. You live at the center for that whole stretch, and you don’t go home each evening.

Six hours of the day are treatment, in therapy sessions and groups. Contingency management (a method that ties small rewards to concrete steps in treatment) is one approach the team uses. There’s a psychiatrist on the team, with case managers and counselors.

The center accepts Medicaid. From July 2025 to June 2026, more than 57 participants stayed in treatment.
