About Transitions Outpatient Program
You'll find Transitions just outside of Skatepark Merriam Park in St. Paul, Minnesota. They have an intensive outpatient program for adults living with addiction and co-occurring mental health disorders. It's a group therapy program that meets five times a week. Groups are structured to follow the same schedule each day and are built around peer support and group therapy. You'll also receive individual support each week. Transitions accept most major insurance plans and Rule 25 funding.
