About Community Bridges Globe Transition Point
The Globe Transition Point in Arizona is an outpatient location of Community Bridges Inc. This is an integrated health center where you can access primary care doctors as well as care for addiction and other specific needs, provided in a cohesive way that addresses all of your needs together.
Health Homes and Outpatient Care
As an integrated health home, this outpatient center provides medical, physical, and behavioral care in one location. We know that we receive a higher level of care when doctors and nurses are able to communicate with each other about our health and even read visit notes. Clients receive primary care, psychiatric and addiction medication management, counseling, case management and peer support.
Primary care offers preventative, wellness, acute sick and chronic health care services for adults. Clients can receive behavioral health care, diabetes care, and immunizations at the same location. When it comes to addiction and mental health care, they have outpatient therapy here as well as intensive outpatient programs which are more structured and thorough.
Crisis Intervention and Stabilization
Crisis intervention and programs for substance use disorders and mental health conditions are available to clients of all ages. You will be stabilized and monitored upon admission. If your crisis is because of withdrawal, the center will provide withdrawal management and medication with a temporary status. Once you have been stabilized you will be referred to a local residential treatment facility.
