About Transformations Wellness Center
A non-profit providing substance use disorder treatment, Transformation Wellness Center is located in Klamath Falls, Oregon. Residential and outpatient treatment is provided, as well as recovering housing. All services are for adult men and women. To be eligible for one of the programs you must have a current ASAM PPC 2R assessment done. If you have a secondary mental health diagnosis, then you will also need a current mental health assessment. If you are eligible, they accept the Oregon Health Plan. Private pay is also accepted. They can provide you with a sliding scale fee if you are a qualified resident.
