Polara Health Cortez Clinic is an outpatient addiction treatment center in Prescott, Arizona offering comprehensive substance use and mental health services to individuals of all ages. Located on South Cortez St, the clinic provides standard outpatient care, intensive outpatient programs and medication assisted treatment for adults, adolescents and young adults.
As the first permanent facility opened by Polara Health in 1984, Cortez Clinic uses a whole person, trauma informed approach to support your recovery and well being.
Personalized Treatment Plans
Adults ages 18 and older can begin treatment with an assessment and step into programs that fit their needs and schedule. The intensive outpatient program includes three hour sessions and one on one support from a dedicated case manager or substance abuse therapist, while the enhanced outpatient program meets three days or nights per week with weekly self help group participation.
Clients receive individual counseling, group therapy and evidence based interventions including cognitive behavioral therapy and motivational interviewing. The clinic also offers medication assisted treatment with Suboxone for opioid use disorder, alongside specialized support for co-occurring mental health conditions, trauma survivors and clients involved in the criminal justice system.
