About Horizon House Delaware County
Horizon House in Swarthmore, Pennsylvania a community organization founded to assist community members who are transitioning from mental health hospitals back into the community. They are accredited by CARF. The organization fills the gaps in mental health care to teach clients how to live and work in a recovery based environment.
Community Based Substance Use Treatment
Assertive Community Treatment (ACT) offers psychiatry, mental health care, drug and alcohol treatment, vocational training, housing and nursing services. They serve community members who have been diagnosed with a serious and persistent mental illness and need additional support outside of traditional mental health services. If you are suffering from a substance use disorder co-occurring with a mental health disorder, the house can provide treatment and services for you.
Employment Assistance
The Community Integrated Employment service here helps clients find employment through employers identified as being a possible match for the client. We know that job hunting can be a daunting task so having an experienced organization who is willing to help us can alleviate stress and discouragement when finding a job.
Mentor Support with a Peer
The Certified Peer Specialist Service assigns you a peer who has experienced similar challenges to you. If you have a mental health and/or substance use disorder, a peer who has been successful in recovery can mentor you as you are trying to regain control of your life. You will set goals together and achieve these goals while your peer mentor supports and roots for you.
Supportive Housing Solutions
Community Residential Rehabilitation Services (CRRS) offers a group home setting for community members with chronic mental illness. Residents must be able to care for themselves, participate in treatment, and not be belligerent with other residents. Meanwhile, Supported Living Services (SLS) provides permanent housing in conjunction with treatment, rehabilitation, and support from community services.
Mobile and Accessible Psychiatric Care
Psychiatric Rehabilitation Services fosters recovery, integration into the community, and improvement of each client’s quality of life. Mental health services are offered at the house or as a mobile option to the client’s home, work, or school. Clients receive assistance and counseling for four aspects of their lives which are living, learning, socializing, and working.
